The programme is concerned with mental illness and the treatment of mental illness. It does not systematically explore every category of mental disorder and the available treatments. Instead the p...rogramme is designed to put the sections of the course text which do deal in detail with disorders and treatments in the context of the wider debate between different schools of thought within psychiatry. More specifically the variety of attitudes to the use of physical methods of treatment in psychiatry.

Footage description: Film sequence shows a patient suffering from severe depression being interviewed by his doctor. Shots of the same patient undergoing electroconvulsive treatment (E.C.T.) Commentary by Rose describes the procedure. Shots of the same patient interviewed after treatment. He appears to have recovered. Dr. William Sargant gives some of the background to the physical treatment of depression and schizophrenia since 1934, He discusses ECT and drug treatments and makes a case in favour of their use. Steven Rose introduces Dr.James Willis who expresses some reservations on the prescriptioin of drugs for cases of depression. Shots of a boy patient discussing his depression with a psychiatrist, Dr.Aaron Esterson. Esterson explains why he is against the use of Jphysical therapy in cases of depression. More shots of the boy patient talking to Esterson. Esterson continues his discussion on depression and his case against physical treatment. He sees institutional psychiatry as a method of social control rather than a healing practice. He touches on the point of civil rights of individual patients being violated - through institutional psychiatry. Dr. James Willis makes a case for physical treatment of depression as this will bring a remission forward in time. He sees this as a justified calculated risk. Dr.William Sargant gives his justification for physical treatment in cases of depression. Steven Rose introduces Dr. Peter Breggin. Peter Breggin discusses the cost of institutions he would like to see set up to treat depression and the approach he would take to treatment. William Sargant continues his justification of physical treatment for depression and schizophrenia. Peter Breggin makes his case against use of physical therapy. Diagrams show how these early operations were carried out. Dr. William Sargant describes some of the early results of lobotomy operations and the improvements in technique which were made. William Sargant argues that improvements in technique make psycho surgery acceptable. Peter Breggin continues his objections to psycho-surgery. He cites lack of research into the results of psycho-surgery. Williwn Sargent continues his argument in favour of psycho surgery. He gives an example of the sort of person he would treat. Peter Breggin explains his approach to treating this sort of person through non-physical means. Steven Rose sums up the case for both sides and discusses the social implication of these treatments.
